if you would be interested in joining and being involved in this please contact the chair, rosemary petty we hope that this website will provide clear and concise assistance, and give our patients the information they require in an easy and convenient format. it has been designed with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us. serving you our dedicated team are here to treat those minor ailments that occur, as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. the technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription or cancel an appointment. partners in care once registered , patients and healthcare professionals work together to ensure the most appropriate care is provided. this partnership philosophy extends even further and our active patient group exists to make sure that patient needs and the practice offering are always heading in the same direction. access & parking the surgery has disabled access and facilities available for its patients. should patients require an interpreter then this can be organised by prior arrangement. there is a large public 'pay and display' car park to the rear of the surgery. please avoid parking in the spaces reserved for the doctors. chapel lane surgery - email address: [email protected] accountable/named gp the practice is required by the government under the terms of the latest gp contract to allocate all patients a named accountable gp. individual patients will be informed of their named accountable gp at the first appropriate interaction with the practice. for convenience if you have a medical card, your accountable gp will be the named doctor you are registered with. patients registering over the last 2-3 years will not have received a medical card as these are no longer routinely sent when registering with a new gp. however, the practice does keep a record of your registered (accountable) gp. if you wish to be told the name of your accountable gp, please ask the receptionists when you are next in the surgery. please note: there is no need to telephone the practice for this information. having a named gp does not prevent you seeing any other doctor in the practice. your named gp will not be available at all times. as you know we operate a signposting and triage service to make sure we can best match patient need. the role of the named gp will not: take on vicarious responsibility for the work of other doctors or health professionals;
